Responsibilities

* Coordinates the provision of hospice services for each patient and family
* Direct and follow established policies and procedures as related to the scope of practice
* Maintain continuity of patient care in a hospice setting
* Proactive, collaborative team member in the hospice environment

LICENSURE, CERTIFICATION, AND EDUCATIONAL REQUIREMENTS
- Current, active, and unrestricted Registered Nurse (RN) licensure in state of practice
