The Position:  Under the supervision of home care RN, the Licensed Vocational Nurse (LVN) provides nursing services to participants in the home setting. The home care LVN follows the plan of care developed in the IDT and provides care within the LVN scope of practice as ordered by the participant’s provider; communicates effectively in the medical record and with all members of the home care team and other program staff to ensure that the participants are receiving care that is appropriate.

Pay rate range for the HomeCare LVN at CEI is $28.14  - $ 42.19 per hr.   Pay rate range based on market for the HomeCare LVN position as well as experience, skills, abilities and work history. **$2,000.00 Sign-on Bonus Offered** 

GENERAL DUTIES and RESPONSIBILITIES: Perform physical evaluation, including vital signs and blood glucose monitoringChart observations of participant’s condition during every visit and in patient health record within required timeframes. Report changes in condition to RN supervisorComplete medication reconciliationPromptly notify providers of changes in participant’s condition including any wounds, physical or behavioral changesAdminister medication, screening tests, and immunizations as prescribedPerform basic wound care as prescribedCoordinate and communicate as needed with RN, occupational therapists, physical therapists, physicians, social workers, dieticians and others on the participant’s care team Report to RN supervisor when objective findings indicate that DME, home care assistance, nutritional services would improve participants quality of life and ability to live in the communityListen to the participant and communicate participant’s questions, concerns and requests to the RN supervisor promptlyEducate participants and their families on home care strategies and proceduresParticipate in department and agency meetings as assignedComplete all required paperwork as assigned within agency time framesParticipate in all agencies’ quality assurance performance improvement program (QAPI) as assignedMaintain participant confidentiality per HIPPA/agency guidelinesFollow all CEI Policies and ProceduresComply with all agency training requirementsMaintain safe working environment by following CEI’s safety P&P’sMaintain a courteous, helpful and professional attitude on the job; display a willingness and ability to be responsive to all customer groupsPerform other duties as assigned in a positive and helpful manner QUALIFICATIONS:

Must be available weekends and evenings to work on call
Certification in Blood Withdrawal and Intravenous Insertion by a CA Board of Vocational Nursing Accredited agency or school Required.Graduate of a school of professional nursingHave a current California Vocational Nurse LicenseA minimum of one (1) year acute experience, and experience working with the frail or elderly populationA minimum of 1 year of work experience as a home care LVN. Two or more years of relevant work experience may be substituted for this requirementA valid California Driver’s license and access to a vehicleExercises initiative and flexibilityMaintains eligibility for licensure by obtaining required continuing education unitsWell organized Ability to effectively problem-solve complex issuesStrong verbal and written communication skills Ability to work as a team player in a multi-cultural, multi-disciplinary settingInterest, enthusiasm and experience working with frail, cognitively impaired older adultsAbility to conscientiously follow directions with limited supervisionStrong interpersonal skills and service orientationAbility to maintain accurate recordsCurrent CPR certification and First Aid training  The above job description is intended to communicate the general function of the mentioned position and by no means shall be considered an exhaustive or complete outline of the specific tasks and functions that will be required.  CEI reserves the right to change job descriptions, site assignments, and or work hours as required by the needs of the program.  All employees are expected to perform their duties within their ability as required by the job and/or as requested by management.
