Work Environment

As a remote position, this role provides flexibility while facilitating crucial mental health care for our patients in hospice. Key Responsibilities
  • Conduct thorough mental health assessments for hospice patients, integrating personal history and unique medical circumstances to create comprehensive care plans.
  • Develop and implement tailored treatment plans that address the psychological and emotional needs of patients, focusing on enhancing their quality of life and facilitating a peaceful transition.
  • Provide evidence-based therapeutic interventions, including grief counseling and psychoeducation, ensuring that patients and their families understand and are supported throughout their experiences.
  • Facilitate individual counseling sessions, utilizing various therapeutic modalities to help patients cope with anxiety, depression, and existential concerns related to their illness.
  • Collaborate with physicians, nurses, and chaplains to ensure an integrated approach to patient care that respects the individual’s values and wishes.
  • Provide crisis intervention and support, utilizing appropriate de-escalation techniques in challenging situations while making qualified referrals when necessary.
  • Educate families about available resources, coping strategies, and the emotional aspects of terminal illness to empower them throughout the care process.
  • Maintain accurate and up-to-date documentation of patient assessments, treatment progress, and intervention outcomes in compliance with professional and legal standards.
Patient Population / Client Focus

You will primarily work with hospice patients in various stages of terminal illnesses, including serious medical conditions that require emotional and psychological support. This unique client focus requires sensitivity to the complexities of end-of-life issues, making your role vital in promoting comfort and awareness for both patients and their families.

Qualifications
  • Master's degree in Social Work from an accredited institution, ensuring a solid foundation in therapeutic practices.
  • Current and valid LCSW licensure in Illinois, verifying your professional qualifications to provide mental health services.
  • A minimum of two years of post-licensure experience, with a strong preference for work in hospice, palliative care, or other health-related settings.
  • A comprehensive understanding of mental health disorders and effective treatment modalities tailored to terminally ill patients.
  • Proficiency in conducting mental health evaluations and creating actionable treatment plans that prioritize individual patient needs.
  • Exceptional interpersonal and communication skills, capable of forming trusting relationships with diverse patient populations.
  • Familiarity with EHR systems and digital communication tools to facilitate efficient documentation and team collaboration.
Preferred Experience
  • Experience in hospice or palliative care settings that specifically cater to the emotional needs of terminally ill patients.
  • Knowledge of community resources available for end-of-life care and bereavement support.
