Supporting the communities of the Central Valley, this Joint Commission Accredited home hospice organization, has provided palliative care residents for over 50 years. Offering a full range of clinical services, from rehabilitation, physical therapy, and disease specific treatments, to palliative and hospice care, this organization and program has proven itself a valuable part of the community.
Reporting to the Administrative Director of Home Health, Palliative, and Hospice Care, the Assistant Nurse Director will play a vital role in the operations of a Hospice Service line. You’ll take clinical and administrative oversight of a compassionate and tightknit team of Home Health RNs, Geriatric Registered Nurses and LVNs, Social Workers, Oncology RN specialists, and support staff. You’ll also get to work with leaders from across the organization and from across borders to develop a clinical continuum of care that will improve the lives of patients on a daily basis.
This is an exciting time to join the leadership team at this esteemed organization, because the Home Palliative Care Director will get to play an integral role in the continued development and growth of this hospice program. Working with the Finance and Operations leaders, you’ll help develop an actionable plan for maintaining current levels of reach, as well as spreading these services to new communities around the Central Valley.
This means taking clinical oversight of a large, diverse team of Hospice and Palliative Care professionals. The Assistant Director of Hospice will have the opportunity to support educational initiatives, to drive quality and efficiency upgrades, and to play a central role in this program.
