CPR certification required in Georgia and preferred elsewhereMust meet all state and regulatory requirements where care is provided. One year of experience in a licensed healthcare setting is required (preferred in California), ideally in hospice, home health, geriatrics, or medical-surgical careMust have a valid driver’s license, reliable vehicle, and state-required liability auto insuranceMust be committed to the organization’s values, mission, and philosophy of care, serving as a role model for team membersPhysical demands include frequent walking, sitting, stooping, driving, typing, and lifting or moving up to 50 lbs; must assist with patient handling up to 200 lbs with helpMust be comfortable working in a variety of conditions, including occasional exposure to outdoor weather, noise, fumes, chemicals, and uneven surfaces Responsibilities:Provide direct nursing care under the guidance of a Hospice RN to support patient comfort, health, and dignity across pre-hospice, continuous care, and in-home respite servicesCollaborate with the interdisciplinary team (IDG), patients, families, and physicians to assess needs, coordinate care, and support holistic well-beingMaintain timely, accurate documentation and communicate any changes or concerns to the clinical team to ensure responsive, high-quality careEducate patients and caregivers in alignment with the plan of care, promoting understanding, confidence, and comfort throughout the care journeyParticipate in on-call rotations, scheduled visits, IDG meetings, and family conferences, contributing to seamless care coordinationRemain current with hospice protocols, licensure requirements, and organizational standards through ongoing training, committee participation, and interdisciplinary collaborationOur Investment in YouCaring for others starts with caring for you.
