Experience:

• 24 months of role experience is required.

• 24 months of Case Management experience is required.

• 3 months of travel experience is required.

• 3 months of experience with Epic is required.

Requirements:

• Candidates must have a New York license in hand at the time of application.

• Travel and Local candidates are eligible to apply for this role.

• COVID vaccination required to apply. Declinations not accepted.

• Flu vaccination required for onboarding. Medical declinations accepted.

Acute care discharge planning experience

SHIFT/HOURS: 8D or 10D (7.5 hrs/day or 9.5 hrs/day) 1-2 weekend days/month

EXPECTED HOURS: 37.5 or 38
