The Crisis Residential Unit LPN is a key associate at Hope Network whose primary responsibilities include: At the direction of the Crisis Residential Unit (CRU) RN, the LPN coordinates and provides direct client care for adults with serious mental illness in a 24/7 licensed facility. Utilizing a Recovery Based process that consist of change through which individuals improve their health and wellness, live a self-directed life, and strive to reach their full potential.  The Crisis Unit LPN works with psychiatrists to ensure good outcomes for persons served, including collaborating with internal and external providers to coordinate psychiatric treatment with medical treatment.  The position manages pharmaceutical and medical supplies and oversees medication monitoring and administration under a psychiatrist or nurse practitioner.  This position promotes recovery-based, person-centered, and culturally competent practices.

 

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

  1. Regular and predictable attendance is an essential requirement of this position.
  2. Provide on-call nursing services and be able to respond in emergency situations, 24/7, as assigned in Crisis Residential Programs (CRU) and within their scope of practice.
  3. Provides professional customer service, always being courteous, responsive, professional, and helpful to the organization’s internal and external customers.
  4. Oversee consumer individualized recovery process relating to their health through support in managing their disease through clinical treatment and providing evidence based medical education.
  5. Embraces the concept of Recovery by providing a stable and safe environment and supporting the engagement of meaningful activities.
  6. Document all encounters and contacts made on behalf of persons served, within designated timeframes.
  7. Provide and/or assist in providing appropriate medical, nursing, or other health care, as needed.
  8. Assist with the assessment of the physical, functional, and psychological needs of persons served, as required.
  9. Maintain consistent collaboration with the registered nurse. 
  10. Act as a proactive and reliable liaison to the psychiatric prescriber. 
  11. Participate in the development of Treatment Plans by assisting persons served in setting goals linked to addressing identified medical and psychiatric needs, as required.
  12. Provide referral to appropriate community resources, facilitate access and communication when multiple services are involved; monitor activities to ensure that services are actually being delivered and meet the needs of the persons served; coordinate services to avoid duplication. 
  13. Maintain accurate records and confidentiality pertaining to persons served. 
  14. Monitor persons served progress toward health care related goal achievements and periodically reassess changes in methods and techniques; coordinate other services as needed.
  15. Identify emergency crisis; participate in immediate clinical assessments and respond accordingly using accepted crisis intervention methods and techniques.
  16. Manage pharmaceuticals and medical supplies.
  17. Assist with health screenings and health assessments as necessary. 
  18. Take the lead role and participate in providing medication administrative management and monitoring. 
  19. Conducts AIMS tests on persons served, as needed.
  20. Prepare data and produce ad-hoc reports in a timely manner, as requested.
  21. Prepare for, and participate in the facilitation of, internal and external audits, as assigned. 
  22. Attend and actively participate in program’s staff team meetings.
  23. Facilitate evidence-based group sessions for persons served. 
  24. Performs any other function deemed appropriate and assigned by the supervisor.Position Qualifications:

 

Position Qualifications:

  1.  State of Michigan LPN licensure.
  2. Valid driver’s license with acceptable driving record according to HN corporate policy
  3. Preferred one or more years of psychiatric nursing experience.
  4. Ability to demonstrate good moral character and even temperament necessary to gain and maintain credibility and trust of persons served and co-workers.
  5. Demonstrated ability to communicate in verbal and written formats to meet position requirements.
  6. Understands the concept of Motivational Interviewing
  7. Familiar with Integrated Care and Duel Diagnosis Treatment (IDDT)
  8. Knowledge about Stages of Change, Trauma Informed Care, Seeking Safety, Family Psycho Education
  9. Ability to schedule/plan work in advance and set effective and realistic timelines to meet goals.
  10. Knowledge of or ability to learn, the use of technological advances that include, but is not limited to: Microsoft Office Suite (Word, Excel, PowerPoint), Internet, fax/copy machines, cell phones, etc. 
  11. Ability to work independently with flexible hours including occasional nights, holidays or weekends to ensure highest level of care for persons served.

 32 hours a week including every other Saturday and Sunday
