Registered Nurse (RN)

Job Requirements:
  • Utilize the nursing process, critical thinking, and problem solving skills to provide holistic care to the individuals, their family members and communities/population across the life span
  • Provide initial and ongoing assessment and interpretation of individuals within the framwork of holistic professional nursing practice
  • Assess/interpret information and data regarding individual and families' to provide appropriate care
  • Assess and interpret the individuals ability to manage their health to transistion their care upon discharge and/or adult placement
  • Identify and communicate needs to the interdisciplanry health care team and other providers as needed
  • Develop, coordinate and update individualized plan of care based on evidence in collaboration with the indivdual's family, interdisciplinary team, and medical home providers to promote continuity of care acros the continuum.
  • Develop and coordinate a comprehensive education plan to meet the individual's and family's needs.
  • Identify, initiate and coordinate therapeutic interventions consistent with the scope of practice and level of competence
  • Implement prescribed interventions and perform independent nursing interventions
  • Interpret medical information inclusive of diagnostic testing and treatment plans to assess individual's response
  • Consistently evaluate the plan of care including individual responses to interventions and achievment of expected outcome and asjust as needed
  • Document complete, accurate, and relevant data for every individual assisged to their care for the shift as well as assigned primary individual caseload
  • Prepare reports and correspondence as required for documenting activites
  • Participate in meetings, conference days, IEP meetings
  • Assist other personnel as required for the purpose of support and completion of work activities
Candidate Profile:
  • Associates Degree in Nursing or higher
  • Current Registered Professional Nurse in the State of NY
  • Current Basic Life Support Certification
  • 2+ years of Nursing experience preferred
    • Experience working with individuals with intellectual or physical disabilities a plus
    • OPWDD strongly preferred
  • Able to demonstrate skill/expertise in utilization of the nursing process
    • Individual-centered medical home and interdisciplinary team approach
  • Experience with SigmaCare and/or E-Clinical Works a plus
  • Maintain required licensing and mandates training by the NYS Education Department
