The Housing Specialist provides supportive services including outreach, monitoring, assessment and service planning, support network building, consultation to families, side-by-side skill training and instruction in the use of community resources, including referrals to community assistance as needed. assumes responsibility for developing, writing, implementing, evaluating, and revising overall treatment goals in collaboration with the client and team; Provides nutrition education and assistance with meal planning, grocery shopping, and food preparation.
Ensures clients have adequate financial support (e.g., Teaches money management skills and assist clients in accessing financial services.
Provides side-by-side support and coaching to help clients socialize (e.g, going with a client to a basketball game).
Plans, structures, and prompts social and leisure-time activities on evenings, weekends, and holidays.
Participates in client-centered comprehensive assessment of psychiatric history, mental status, physical health, use of drugs or alcohol, education and employment, social development and functioning, activities of daily living, and family structure and relationships.
A Bachelor’s degree with major course work in criminal justice sociology, social work, psychology, counseling or education; PA Child Abuse/Criminal Clearances and FBI Background check required.
TRAINING AND EXPERIENCE:
Must understand the principles of mental illness and ability to implement them, and behavioral best practice and crisis management planning.
