JOB SUMMARY As a Community Health Worker for Community Clinical Services (CCS), this position is a trained and trusted public health worker who is respected by the people served and applies his/her unique understanding of the experience, socio-economic needs, language, and/or culture of the communities served to:# # Act as a bridge between providers and individuals to promote health, reduce disparities, and improve service delivery; and# # Advocate for individual and community needs. CCS is a federally qualified health center providing primary care, pediatric dental, and behavioral health services to patients regardless of ability to pay. # Scope of Service: The CHW is responsible for extending the reach of Community Clinical Services in the local community. The CHW engages community members around their health needs, focusing on cancer screenings and follow-up; identifies and refers patients to other local resources, serves as a cultural broker for program services, supports patients in accessing health care, empowers clients to develop skills for navigating the health system, and teaches clients how to adopt healthy behaviors. The CHW works closely to support other staff responsible for the coordination and delivery of medical services to patients. The majority of work may occur within the community. # JOB REQUIREMENTS EDUCATION High School or GED required Completion of Maine Core Competency Training for Community Health Workers preferred EXPERIENCE 1-year experience OTHER SKILLS REQUIRED Fluency in English and Portuguese (both written and verbal). Fluency in additional languages is preferred but not required. Current driver#s license, with a good driving record for the past 3 years, or period of licensure, whichever is shorter. JOB SUMMARY

JOB REQUIREMENTS

EDUCATION

* High School or GED required
* Completion of Maine Core Competency Training for Community Health Workers preferred

EXPERIENCE

* 1-year experience
