CCALAC AmeriCorps Health Fellows CCALAC represents non-profit community clinics that operate primary care sites throughout the county. Our 66 member organizations operate over 300 sites throughout Los Angeles and serve as the medical home for nearly 1.5 million patients each year. Community clinics provide primary health care, including medical, dental, and mental health services to the uninsured, under-insured, low-income, high-risk and vulnerable populations. They serve all, regardless of ability to pay. CCALAC's central role is to help our members serve their patients in an efficient and cost-effective manner while providing quality care. CCALAC places AmeriCorps Health Fellows at CCALAC member health centers across LA County. The program is specifically focused on community health, providing access to primary care and health services to LA County's under resourced population. Positions Available are as follows: Full-Time AmeriCorps Health Fellows serve 1,700 hours (40 hours a week) Part-Time AmeriCorps Health Fellows serve 900 hours (30-35 hours a week) AmeriCorps Recruitment Capacity (ARC) Member: serve 1,700 hours (40 hours a week) These positions are held at CCALAC member clinics in exchange for a living allowance, education award, work experience, the opportunity to give back to the community and employment after the successful completion of the program. AmeriCorps Health Fellows do not provide health services related to the prohibited activities (including abortion referrals or services). The AmeriCorps Recruitment Capacity member has the same benefits as an AmeriCorps Health Fellow. The ARC Member would also work directly with the AmeriCorps Program Manager to develop and foster partnerships to increase outreach to potential AmeriCorps Members, build social media presence through active posting, develop marketing materials and attend college and career fairs, etc. Recruitment and retention related duties. Member Duties : 1) Outreach: Conduct outreach to new/existing patients at: health fairs, community organizations, waiting/exam rooms, universities, and community events to inform/educate about primary care services. 2) Enrollment: Assist patients with the enrollment process into health programs, health insurance plans and/or pharmaceutical assistance programs. 3) Case Management: Provide support for patients in navigating health systems, service and programs. This includes assisting patients with the Patient Portal, translation, program referrals, housing and food security, and other social determinants of health. 4) Health Education: Provide linguistically and culturally appropriate health education in one-on-one or group setting. Engage and educate patients to improve health outcomes in chronic disease control. AmeriCorps Health Fellows do not provide health services related to the prohibited activities, including abortion referrals or services.
