 Primary Responsibilities:

 · Assist with daily living activities such as serving meals, feeding, and dressing residents.

· Assist with lifting, ambulating, turning, and positioning residents, and helping them into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc., while following resident-specific safety needs.

· Help prepare residents for transporting or getting back from appointments, activities, and social programs.

· Track and document actions by completing forms, reports, logs, and records in a timely fashion.

· Review care plans routinely as residents’ needs change.

· Participate in and receive nursing report at beginning of shift and during shift change.

· Care for residents’ personal needs & hygiene by giving baths, back rubs, shampoos, shaves, assisting with bathroom trips, and changing bedpans and/or urinals.

· Understand duties, responsibilities, and activities may change or be assigned at any time without notice.
