Northern Riverview Assisted Living Center is looking for a RN Case Manager Supervisor to work full-time in Havenstraw, NY 

Position Details & Duties:

  • The Case Manager position is office based.
  • The Case Manager coordinates and organizes the resources & services of the assigned case load; working with the patient, family, physician, and Field RNs.

Requirements Include:

  • Valid NYS Registered Nurses license
  • Minimum 3 yrs. relevant CHHA experience
  • Strong computer skills
  • Experience with Electronic Medical Records preferred
