CLINICAL PHARMACIST SPECIALIST- AMBULATORY CARE - (Temple University Hospital/40 hours days)

The Clinical Pharmacist Specialist Ambulatory Care is responsible for providing integrated, accessible healthcare services by being responsible for managing patients' medication use, creating sustainable relationships with patients their family and the community. Provide direct patient care and medication management for ambulatory patients, coordinate care, serve as the patients' advocate, and promote health and wellness. Triage and refer patients to the appropriate healthcare provider. Provide patient education and promote self management. Develop collaborative practice agreements and practice within the structure of those agreements. Train pharmacy students and pharmacy practice residents. Provide in-service education and drug information to healthcare professionals. Education Doctorate Degree : PharmD (Required) Other : PGY2 Pharmacy Residency in Ambulatory Care (Required) Combination of relevant education and experience may be considered in lieu of degree. Experience 2 years experience in Ambulatory Care Pharmacy (Preferred) License/Certifications RPH-LIC - PA Pharmacist License (Required) RPI-LIC - Auth to Administer Injectibles (Preferred) _ '234959
