Job Details

We are seeking a compassionate and dedicated Registered Nurse (RN) Case Manager with a specialty in Hospice care to join our dynamic healthcare team.

The ideal candidate will be responsible for the coordination of care for a set group of patients to ensure the highest quality of holistic care is provided. This is a full-time permanent position that requires a unique blend of clinical expertise, patient advocacy, and emotional support. This role is an opportunity to make a significant and meaningful impact in the lives of patients and their families during their most challenging times.

Responsibilities

  • Provide comprehensive nursing care to patients
  • Coordinate the development and implementation of individualized patient care plans in collaboration with the interdisciplinary team.
  • Regularly assess the patient's condition, monitor vital signs, and adjust care plans as necessary.
  • Provide emotional support and counseling to patients and their families, helping them to navigate end-of-life decisions and challenges.
  • Liaise with physicians, therapists, and other healthcare professionals to ensure the seamless delivery of care.
  • Educate patients and their families on disease management, self-care techniques, end-of-life care, and the bereavement process.
  • Document patient information, care provided, and any changes in patient condition in a timely and accurate manner.
  • Ensure compliance with all federal and state hospice regulatory requirements.
  • Participate in on-call rotation to provide 24/7 patient support.

Qualifications

  • Current and unencumbered Registered Nurse (RN) license to practice in California
  • Ideally a minimum of 2 years of nursing experience, with at least one year in hospice or palliative care.
  • Experience in case management and development of individualized care plans.
  • Excellent clinical nursing skills and a deep understanding of the hospice philosophy.
  • Exceptional communication and interpersonal skills to provide comfort and support to patients and their families.
  • Ability to work independently, manage time effectively, and prioritize tasks.
  • Proficiency in electronic medical records (EMR) systems and Microsoft Office Suite.
  • Current CPR certification.
