Several Immediate PRN positions available to RNs with Home Care Experience.

Our client is a Medicare certified and Pennsylvania state-licensed home health care providing individualized skilled care in the comfort of your home is looking for an Home Care Registered Nurse

Type: Contract

Schedule: Day shift, Monday to Friday, Weekends as needed

Location: Philadelphia, PA

Duties:

Follow patient's plan of care in accordance to doctor's orders

Travel to patient homes as assigned and manage own schedule

Home care visit duties may include: medication administration, wound care, catheter changes, disease management and education

Monitor, observe, evaluate and manage patients' conditions, report to physician and properly document

Perform patient assessments, evaluations and Start of Care Admissions

Case manage, follow up and coordinate with care team, family and physician

Qualifications:

A current license as a Registered Nurse in good standing and able to practice in Pennsylvania.

A minimum of one year of recent, verifiable RN clinical (medical/surgical) experience.

A Minimum one year prior home health care experience providing home visits

Graduation from an accredited and approved nursing program

Strong interpersonal skills, excellent work ethic and communication skills

Experience with electronic medical records (EMR) preferred. Knowledge of Axxess software is a plus.

Knowledge of OASIS documentation

Knowledge of Medicare requirements and compliance

Current CPR certification

Clear criminal background history and clear drug screen

Associate (Required)

2 years of Home Health Care Experience required

1 year of Acute care experience preferred

1 year of Nursing experience required
