Job Summary

The Clinical Transport Nurse provides professional nursing care through high-quality assessment, triage / treatment to assess the nature and extent of a patient's illness / injury and prioritize the needed care. Provides comprehensive nursing care through skillful assessment, diagnosis, outcomes identification, planning, implementation, and evaluation. Patients often are intubated, on ventilators, or have hemodynamic instability. The Clinical Transport Nurse performs invasive procedures and hemodynamic management of critical patients. Manges ventilator settings, vasopressors, and solves complex clinical challenges. The Clinical Transport Nurse must have strong clinical knowledge and skills necessary to care for patients of all ages with a diverse range of medical needs (neonatal - geriatric) operating specialized medical equipment and utilizing advanced skills. The Clinical Transport Nurse functions autonomously outside of the hospital environment under standards and protocols approved by the Medical Director as established by the North Carolina Board of Nursing, North Carolina Offices of Emergency Services, South Carolina Department of Health and Environmental Control. The Clinical Transport Nurse must be proficient in advanced practice procedures to provide improved patient outcomes.

A registered nurse's practice is guided by the ANA Code of Ethics for Nurses with Imperative Statements.

Essential Functions

* Collects data pertinent to the healthcare consumer's health or the situation.
* Analyzes the assessment data to determine the diagnosis or issues.
* Identifies expected outcomes for a plan individualized to the healthcare consumer or the situation
* Develops a plan that prescribes strategies and alternatives to attain expected outcomes.
* Implements the identified plan, coordinates care delivery, and employs strategies to promote health and a safe environment.
* Evaluates progress toward attainment of outcomes.
* Practices ethically
* Delegates elements of care to appropriate healthcare workers
* Promotes shared governance, or facility decision making activities, developing and nurturing research to positively affect clinical outcomes and promotion
* Attains knowledge and competence that reflects the current nursing practice

* Required to have operational knowledge for multiple modes of transportation including ambulances, helicopters and airplanes. Must be trained in aviation and meet the minimum requirements to operate as a "flight crew member" per FAA Part 135 regulations. Including operation of Night Vision Googles to ensure safe landings.
* Independently initiates advanced practice procedures to provide improved patient outcomes such as: drug assisted intubation, video laryngoscopy, needle chest decompression, chest tube insertion, central line placement (UA/UV, EJ), intraosseous placement, transvenous/transcutaneous pacing, needle cricothyroidotomy, surgical cricothyroidotomy, and initiation/titration of pharmacological agents.
* Initiates intubation procedures and sets up ventilation equipment. Operates, assesses, and manages patient airway support, ventilator settings and equipment.
* Transports and monitors patients requiring ECMO, Medical Circulatory Devices (VAD, Impella), Intra-aortic Balloon Counter Pulsation, Internal/External Pacing, Ventriculostomy, and Point of Care. Must be capable of handling equipment challenges and failures without support of internal hospital resources.
* Works in an autonomous environment that requires patient management and critical decision-making based upon the dynamic patient status in unique settings such as accident/scene calls outside the traditional hospital and healthcare setting with limited resources and support.
* Responds to accident scenes and is expected to work in hazardous environments. Including inclement weather, biohazards and highly infectious patients. Responds to disasters and maybe deployed for extended periods of time.

Physical Requirements

* In order to meet safe flight weight limitations and scheduling requirements candidates must weigh less than 240 pounds and must maintain that weight while employed at MCA (exception will be made only when department has ground position needs).
* Duties require working and transporting patients in ambulances as well as driving vehicles including ambulances in all weather conditions. Required to drive to and work at multiple locations across NC and SC based on staffing needs.
* Must not have any physical, emotional, or mental impairment to prevent them from riding, driving, or administering patient care in transport vehicle. This work area has cramped space and can be poorly lit. Noise levels are moderate as associated with working in and around emergency vehicles. Hearing protection is available and required for use around "hot" aircraft. Requires handling of patients that may have infectious disease, possible exposure to unpleasant conditions and / or handling of contaminated materials or patients.
* Heavy lifting required, with the assistance of a partner, to load patients, unload patients, and carry equipment. Good physical stamina and endurance is necessary to perform duties. Teammate must maintain fitness standards in accordance with Policy 2.210 (Fit for Duty).
* Must have intact sense of sight, hearing, smell, and touch. Finger dexterity, good motor, and eye-hand coordination required. Requires good personal hygiene and appropriate conduct. Must have excellent communication and interpersonal skills to include, but not limited to speaking English in good, understandable terms.
* Additional department specific physical requirements may be identified for unique responsibilities within the department by the nurse leader.

Education, Experience and Certifications

* Graduate from an accredited school of nursing. BSN preferred.
* Complies with the Clinical Nurse requirements of the professional nurse clinical advancement program. Experience as outlined in the Professional Nurse Clinical Advancement Program.
* Current RN license or temporary license as a registered nurse petitioner in the state in which you work and reside or if declaring a National License Compact (NLC) State as your primary State of residency, meet the licensure requirements in your home state: or for Non-National License Compact states, current RN licensed or temporary License as a Registered Nurse petitioner required in the state where the RN works.
* Minimum of three (3) years clinical nursing experience in the Emergency Department and/or Intensive Care Unit required.
* Demonstrate proficiency in advanced procedures and skills upon completion of orientation and bi-annually.
* EMT certification upon hire or by the end of orientation.
* Paramedic Certification required within one year of completing orientation. Must acquire NC EMT-P, SC EMT-P and National Registry. Must be maintained throughout employment.*
* Completion of an approved trauma certification within one year of hire and must be maintained throughout employment.
* BLS, ACLS, PALS, NRP and S.T.A.B.L.E. required upon completion of orientation and must be maintained throughout employment.
* Advanced certification such as a certified flight registered nurse (CFRN), certified transport registered nurse (CTRN), certified emergency nurse (CEN), certified critical care registered (CCRN), or other equivalent as approved required to maintain position according to standards of commission on accreditation of medical transport systems.
* Must maintain annual compliance in education related to specialty area and/or primary base location outside of typical nursing practice.
