I. Position: Operating Room Registered Nurse (Circulator)

II. Qualifications: The Operating Room Registered Nurse (Circulator) must be licensed in the state of hire/practice.

III. Purpose and Scope: The Operating Room Nurse or RN Circulator oversees the operating room environment. When sterile procedures are performed in the OR suite, the Operating Room RN is responsible for planning, coordinating, controlling, and carrying out direct and indirect patient care activities that support the medical program of care and are based on actual and potential patient needs. The RN Circulator functions outside the sterile field during the operation. It assists the physicians and the sterile team by anticipating and providing needed supplies and equipment, adjusting the lights and other electrical equipment, relaying messages, and running errands. The Circulating RN plays a vital role in facilitating the performance of the operative procedure through alert observation, assessment, and recognition of needs, ability to set priorities, initiative, sound judgment, and competence in coordinating and carrying out activities in his/her operating room. This person observes and enforces strict asepsis standards, controlling traffic and environmental cleanliness in the room.

The Registered Professional Nurse in the Operating Room may also function as a “Scrub Person.”

IV. Reports to Nurse Manager and/or Director of Nursing

V. Responsibilities: Is responsible and accountable for own nursing practice.

Patient Care/Clinical Practice

  1. Meets standards for job performance of Operating Room Circulating Registered Nurse.
  2. May be required to meet standards for job performance of Scrub Person.
  3. Is responsible and accountable for the perioperative nursing process as defined by the ANA, which consists of the six steps of assessment, nursing diagnosis, outcome identification, planning, implementation, and evaluation of perioperative nursing care for adult, geriatric, and pediatric patients. Provides unique age-related needs from admission through discharge.
  4. Establishes, implements, and monitors, in conjunction with other patient care team members, standards of patient care within the framework of this facility's standards. Evaluate, coordinate, and revise the care plan using patient/family and health care team information.
  5. Directs unlicensed assistive personnel / peri-operative team towards safe and competent patient care.
  6. Administers medications correctly within the restrictions and limitations defined by the State Practice Act and this facility’s policies and procedures
  7. Implements and ensures physician orders are performed, including orders delegated to other health care professionals.
  8. Maintains surgeon preference case data and instrument and equipment inventory. Oversees appropriate supply and equipment selection for the planned surgical procedures and assures that cases are pulled and rooms are set up accurately.
  9. Accurately monitors and documents facility logs, ensuring those regulatory requirements are met.
  10. Executes the operative schedule in a safe and quality manner demonstrating flexibility with necessary changes and arrangements for added operations, cancellations, or deviations in plan of care.
  11. Applies sound nursing principles to organize, plan, coordinate, and control the flow of activities and resource utilization. Demonstrates cognizance of Center policies, procedures, and job descriptions.
  12. Demonstrates analytical abilities necessary to identify a problem and develop a sound action plan for resolution.
  13. Actively participates in the Center’s quality / performance improvement monitoring and assists with preparation of these presentations and reports.
  14. Works flexible hours in assuming responsibility and accountability for providing patient care.
  15. Assumes roles as circulating nurse, scrub person as requested, and is able to assume job roles for other peri-operative non-licensed team members.
  16. Ensures a safe environment for all patients, families, and staff.
  17. Changes gas cylinders as necessary to maintain medical gas system integrity. Ensures communication of need for replacement tanks or system problems/difficulties.
  18. Seeks learning opportunities and demonstrates skill level and capabilities as a member of the center's peri-operative staff, contributing to success measures for quality outcomes.
  19. Promotes cooperation and minimizes disruption to the working environment
  20. Prepares Center nursing and operative documentation, charges, and billing data concerns for each patient.
  21. Reviews informed consent and documents, identifies the operative site, maintains fire safety parameters, and demonstrates knowledge regarding patient positioning using appropriate devices and padding.
  22. Performs job responsibilities associated with the soiled and clean utility rooms for instrument and equipment cleaning and sterilization.
  23. Assists in PACU, Recovery 2, Preop Holding, and Treatment Room as needed.
