Position Summary
ChesPenn Health Services is seeking a community health-focused Patient Engagement Specialist to conduct patient outreach and engagement activities and assist with Quality Improvement and Program Development in our network of community health centers.
Qualifications
- Bachelor's Degree in health science-related field, MPH preferred
- 1 to 2 years' experience preferred
- Current COVID-19 and flu vaccines required
Knowledge, Skills and Abilities
- Experience and knowledge in the design, implementation and evaluation of community health programs and similar initiatives to improve the health outcomes of a target population preferred
- Demonstrates strong interpersonal skills with the emphasis on team development and facilitation techniques.
- Good written and verbal communication skills in English with an emphasis on presentation skills to patient, clinical provider, and administrative staff.
- Demonstrates ability to manage multiple projects, meet deadlines and adjust to changes in a fast-paced, demanding environment.
- Demonstrates experience with MS Office 365: Outlook, Word, Excel, Access and Power Point
- Self-motivated and able to work independently.
- Possession of a valid driver's license and licensed automobile
- Ability to meet approved minimal physical and medical standards.
- Command of EMR, CPS and population health software to enable coordination of patient care. Preferred
What You'll Do
- Implement key quality improvement activities and projects in support of the organization's strategic goals and federal requirements
- Assist in the coordination and development of the organization's Quality Improvement program
- Conduct outreach to patients via phone, text, email, and in person to engage patient and coordinate care to ensure care gaps or other needs are being addressed.
- Educate and coordinate patient care; assist patients in navigating necessary services such as laboratory, specialist care, insurance and other social services.
- Assist in the coordination, development and implementation of key organizational QI activities, support organizational strategic goals and meeting federal and grant-funded program requirements.
- Assist in analysis and translation of population health data and performance metrics into digestible reports to enable upper management to make informed strategic decisions.
- Collect, manage, and validate patient level data to develop strategies for the effective engagement of patient to address health care gaps.
- Work directly with clinical staff in developing and implementing QI interventions.
- Travel among sites, within the communities served by ChesPenn and to local and national meetings and conference as needed
- Work directly with Operations Management on revenue enhancement projects involving review of patient charts and billing data
