Function:

* Receives, reviews and coordinates patient clinical referral activity.
* Completes admission assessments
* Serve as Charge Nurse on short term rehab unit
* Inputting physician orders in system
* Assists with arrangements and confirmation of all admissions.
* Coordinates the assignment of attending physician.
* Conducts discharge assessments
* Completes discharge packets
* Conducts immunizations and covid testing for new residents
* Promotes optimal communication among program staff members, facility staff, referrals sources, physiciansand patients.
* Adheres to all safety and risk management protocols and programs.
* Participates in direct and indirect patient care activities as warranted.
* Participates in quality improvement activities and QAPI process to increase efficiency through improved systems and processes
* Exhibits courtesy, compassion and respect to patients, families, visitors, physicians, administrators and coworkers.
* Performs other duties or functions as directed.

Responsibilities:

This position handles review of clinical data on referrals, assesses new admits, completes EMR data entry.

Requirements:

* High School graduate.
* If RN, must have a current license to practice profession in state.
* Must have a reliable source of transportation.
* Valid driver's license and proof of insurance.
* Ability to understand, remember and carry out oral and written instructions in English.
* Ability to work in Excel, Word, Power Point, PCC and Nexion dashboard or have the ability to learn.
* Ability to speak clearly and hear well enough to communicate with others.
* Ability to complete assignments while having constant interruptions.
* Must be capable of performing the Essential Job Functions of this job, with or without reasonable accommodation.
* At least one year of office or LTC clerical experience.
* Knowledge of medical terminology preferred.
* Ability to type.
* Prior experience in admissions preferred.
