Position Summary
The RN Annual Wellness Visit Nurse will deliver care to patients and families with needs ranging from basic to complex utilizing the nursing process, creating a therapeutic and caring environment for patient and families. The incumbent practices in a manner consistent with the Elliot Hospital Interprofessional Practice model; the ANA's Social Policy Statement, Scope and Standards of Practice, and the Nursing Code of Ethics; and relevant specialty standards of practice. This position will be primarily responsible for performing the Annual Wellness Visits for Medicare eligible consumers, within CMS guidelines, in scheduled primary care offices, which would include all documentation and scanning of related documents to complete the visit, as assigned.
Primary duties and Responsibilities
* Embrace and promote change to support optimal patient outcomes and an effective professional practice environment.
* Involve patient/family in goal setting and planning and communicate that plan of care to patient and family for input as well as to colleagues to ensure seamless care across shifts and settings.
* Demonstrate accountability for continued professional growth and development.
* Demonstrate accountability for own role in smooth operations of unit.
* Demonstrate the ability to recognize and manage rapidly changing situations.
* Utilize current evidence and relevant national/specialty standards to guide practice and/or performance and identifies areas for improvement.
Position Qualifications
Education
* Graduate of an accredited school of nursing required, with a commitment to obtain BSN within five (5) years of hire required.
* BSN preferred at time of hire.
Licensure/certification
* New Hampshire Registered Nurse or Compact State Registered Nurse License required.
* Ability to obtain BCLS qualification during orientation period and maintain throughout the duration of appointment in position required.
* Nurses are expected to pursue professional certification upon eligibility or within three (3) years of entering the specialty practice.
