Summary:

The incumbent is responsible for developing, coordinating, implementing, teaching and evaluating a diabetes education program for patients and their families served at Northern Light Health and for the broader community, and for the organization when reviewing standards of practice. The incumbent is responsible for the nutritional care of adult diabetes outpatients with moderate to high nutrition risk, severe disease and/or multiple, medical problems. Duties include assessment of nutritional status, recommendations on clinical efficacy of diet plans, development and implementation of care plans, and nutrition counseling. The incumbent is also responsible for overseeing the day-to-day operations of the diabetes self-management education program at all sites. The program coordinator ensures that the National Standards for Diabetes Self-Management Education (DSME) and Support are followed at all times, including data tracking and reporting of outcome data. The program coordinator must also meet and maintain the requirements of the American Diabetes Association Education Recognition Program at all times. The incumbent may have access to highly confidential patient, employee and/or organizational proprietary information, and must handle and protect the information in accordance with hospital and system policies, HIPAA requirements and the highest level of ethical standards. The incumbent is responsible for reporting all security events, potential events, or other security risks to the organization. The incumbent is responsible for participation in and completion of all patient safety initiatives appropriate to the position, and conducts all job responsibilities according to the Mission and Values of Northern Light Health.

* Documents visit appropriately and completely within 48 hours of visit.
* Completes all forms in accordance with department and hospital policy and regulatory requirements.

* Completes monthly organization assignment each month and documents on form.
* Down time is used productively.
* Effectively communicates with others (including patients, families, staff, providers).
* Maintains patient confidentiality at all times.
* Reports concerns/issues to supervisor.
* Handwriting is legible.
* Is an effective patient/family group leader.
* Is proficient and competent in the use and care of equipment within the department.
* Maintains or works towards CDE certification.
* Maintains licensure as Registered, Licensed Dietitian or Registered Nurse or eligible.
* Identifies individual patient needs and develops appropriate treatment/education plan.
* Submits accurate charges for visits and POC testing within 48 hours of visit.
* Makes suggestions to improve operational efficiency and cost control.
* Provides supervisors information on expenses for the next fiscal year when requested.
* Willing to accept patient add-ons to schedule.
