Our Team
Located at 195 Worcester Street and 978 Worcester Street Wellesley, MA, our Internal Medicine/Family Medicine practices deliver a full range of preventive and health maintenance care. We not only treat a range of acute and chronic health problems, but we also coordinate our patients' care when they need treatment by specialists.

The Opportunity
We are seeking a Full- Time, 40-hour LPN to support out two primary care practices onsite Monday through Friday from 8:30am to 5:00pm. This onsite position is split between our two offices, 195 Worcester Street, and 978 Worcester Street in Wellesley.
Dedicated to clinical support, our nurses provide professional nursing care while collaborating with providers, staff, patients and families. This role is a mix of both patient facing and non-patient facing responsibilities.
Responsibilities:
* Responds to and prioritize incoming telephone calls, and triage appropriately.
* Communicates with patients and family regarding medications, test results, and treatment instructions.
* Vaccine/Medication administration and Management -Vaccine inventory
* Population Health Management/Health Maintenance- Chronic Disease mgmt.
* Observes, records and reports patient's condition and plan of care to the physicians/provider.
* Manages all aspects of process Prior Authorizations for: Rxs, DMEs, procedures, labs.
* In-Basket management.
* Ownership of Nurse Resource schedule for vaccines, BP checks, etc
Qualifications

* Graduate of an approved LPN / Licensed Practical Nurse program (per state requirements)
* Current state licensed as an LPN / Licensed Practical Nurse (as required by state regulation)
* Current CPR certification
