Job Title: Speech and Language Pathologist (SLP)


Reports To: Director of Rehabilitation


Job Description: The primary responsibility of the Speech and Language Pathologist (SLP) is to prevent, evaluate, assess, diagnose, and treat speech, language, communication, and swallowing disorders as ordered by a physician.


Job Duties:


• Provide skilled, medically necessary speech and language therapy assessments as well as develop and implement individualized treatment plans to increase resident’s independence and functioning.


• Review the available resident information and evaluate the impact of this information on the assessment and treatment process.


• Conduct and document a thorough functional assessment of the resident, utilizing standardized procedures.


• Develop and implement an individualized treatment plan based on resident needs and containing achievable functional goals.


• Comply with evaluation, treatment, and documentation guidelines and consistently maintain timely and accurate department records.


• Revise treatment plans, as needed, throughout the treatment process.


• Enter treatment encounter note in the resident record following each treatment session.


• Conducts screenings of residents at regular intervals to determine need for intervention/treatment.


• Coordinate and collaborate with other professionals for diagnostic swallowing procedures (MBSS) as ordered by a physician.


• Provide aural rehab for residents who are deaf or hard of hearing


• Provide augmentative and alternative systems for individuals with severe expressive and/or language comprehension disorders.


• Supervise, as appropriate, provisional licensees, students including clinical fellowship year (CFY) in accordance with state regulations.


• Interprets and communicates evaluation findings to patients, family, and other team members.


• Establishes individual treatment schedules in conjunction with other team members.


• Provide training and education to client, family, and caregivers and other professionals


• Demonstrate competent technical skills according to current Speech and Language Pathology standards of practice.


• Communicate changes in patient condition.


• Participate in collaborative care conferences and discharge planning.


• Provide accurate and timely billing records and supporting clinical documentation.


• Demonstrate a knowledge of OBRA regulations, HIPAA, Local Coverage Determination, and third-party reimbursement guidelines.


• Complies with all Infection Control, Universal Precautions, and OSHA standards for the healthcare professional.


• Assists with and participates in Quality Assurance program as required.


• Document the course of treatment in a clear, concise, discharge summary utilizing a prescribed format.


• Orient residents to program services and treatment procedures appropriate to their needs. Inform residents of any potential risk during procedures.


• Work with nursing staff, resident, and families to maximize resident’s use of functional abilities.


• Instruct residents and family/other caregiver about exercise programs and therapeutic procedures to be continued by the resident following treatment.


• Assist in the referral process when a resident requires additional services following discharge.


• Attend and participate in or provide information for rehabilitation team conferences regarding resident progress, problems, or needs.


• Attend and participate in other department/facility meetings, as required.


• Access continuing education opportunities appropriate to discipline to maintain clinical competency.


• Participate in quality improvement process.


• Act in compliance with regulatory and professional standards and guidelines.


• Adhere to facility policies and procedures and participate in facility quality improvement and safety programs.


• Understand reimbursement by various payers and allocates therapy resources efficiently in accordance with resident goals and outcomes related to discharge destination.


• Demonstrate a knowledge of OBRA regulations, HIPAA, Local Coverage Determination, and third party reimbursement guidelines.


• Performs other duties as assigned
