Responsibilities

In your job, you will have the ability to make a difference in a patient's journey by:

* Work under direct supervision and in tandem with the Oncology Nurse Navigator to coordinate patient care through all phases of the care continuum.
* Support the Oncology Nurse Navigator in reporting patient and family challenges and assist with potential solutions, while considering the patient's values and cultural, spiritual, and social needs.
* Coordinate patient needs for appointments and schedule appropriately to expedite the plan of care.
* Ensure closure of all referral loops, internal and external, through procurement and inclusion of referral documentation in the medical record in a timely manner.
* Appropriately document all patient interactions and services provided within the electronic medical record.
* Act as a liaison and advocate for patients, families and caregivers when assisting the multidisciplinary team.
* Serve as a liaison/contact for referring physicians, external medical offices, and patients in a timely, professional manner.
* Participate in continuous process improvement of patient outcomes, assuring appropriate utilization of resources to increase patient, family, and caregiver satisfaction.
* May schedule outpatient appointments/testing/surgeries and hospital admissions upon request.
* Obtain necessary pre-certification as required.
* May communicate to patient about surgeries or tests being scheduled and potential prep work needed, inquiring about test specifics from necessary parties and gathering patient information as needed.
* May be responsible for follow up on all pre-operative tests to ensure cleared for surgical procedure and communicates with physician and patients if further testing is required before surgery is performed.
* Where applicable, basic understanding of medical terminology for tests and surgical procedures, including coding and ability to look up codes specific to diagnosis, surgery, and scheduling procedure.

Qualifications

Minimum Qualifications:

* High School Diploma or equivalent; advanced education preferred.
* Minimum four (4) years of experience working in a clinical health care environment.
* Oncology-related education, planning, management, or similar experience is required.

What You Bring to the Team:

* Medical Assistant (certified) experience with medical, radiation, and /or surgical oncology/hematology patient population preferred.
* Strong organizational and interpersonal skills.
* Strong knowledge and proficient use of an electronic medical record (EMR) system.
* Proficiency in reviewing medical records and other patient care information.
* Strong knowledge and understanding of HIPAA compliance and patient confidentiality regulations and policies.
* Ability to develop collaborative relationships and work in teams, including medical, surgical, and radiation oncologists and referring providers.
* Strong written and verbal communication skills, including medical terminology and therapeutic interventions available.
* Strong customer service skills.
* Knowledge of local community resources.
