Registered Nurse Case Manager Job Summary

We are looking for an experienced and compassionate Registered Nurse Case Manager to join our Tarlani Healthcare team. The ideal candidate will coordinate and manage the individualized care of patients by ensuring that the services they receive are integrated, comprehensive, and appropriate. The Reregistered Nurse Case Manager will assess, plan, coordinate, monitor, and evaluate the options and services required to meet the patient’s home health service needs.


Responsibilities

  • Develop individualized care plans for patients by assessing the patient’s physical, psychological, social, and economic needs;
  • Conducting intake assessments with patients and families;
  • Adjusting treatment protocols, as needed;
  • Develop discharge plans for patients, including arrangements for follow-up care and home health services;
  • Monitor and coordinate services to ensure quality patient care;
  • Monitor the effectiveness of care plans and make necessary changes;
  • Ensuring that families feel sufficiently equipped and supported;
  • Suggesting and coordinating appropriate follow-up services to promote wellness;
  • Serve as a liaison between patients, families, healthcare providers, and payers;
  • Promote continuity of care by advocating for the patient’s needs;
  • Advise and educate patients and families on available services, care plans, and health goals;
  • Document patient care plans, progress notes, and outcomes;



Qualifications

  • Bachelor’s degree in Nursing;
  • Advanced Degree in Nursing is strongly advantageous;
  • Licensure as a Registered Nurse to practice in the State of California;
  • 3+ years experience as a Registered Nurse Case Manager or similar role;
  • Knowledge of healthcare and insurance systems;
  • Familiarity with patient database and EMR systems;
  • Excellent communication, interpersonal, and organizational skills;
  • Able to multitask, prioritize, and manage time effectively;
  • Strong understanding of ethics surrounding the storage of patient information;
  • Exceptional leadership, supervision, and motivational skills;
  • Committed to promoting patients' well-being;
  • Ability to work evenings, weekends, and holidays if necessary;
