Currently, we are seeking a dedicated Licensed Vocational Nurse (LVN) to join our team on a temporary basis to cover at our day program facility in Santa Rosa.

Job Responsibilities:

* Provide nursing care and support to individuals with diverse healthcare needs at our day program facility.
* Administer medications and treatments according to individual care plans and physician orders.
* Monitor and assess the health status of program participants and promptly respond to any changes or concerns.
* Collaborate with other healthcare professionals and support staff to ensure comprehensive care delivery.
* Maintain accurate and up-to-date documentation of nursing assessments, interventions, and outcomes.

Qualifications:

* Valid California LVN license.
* Prior experience working with individuals with special healthcare needs preferred.
* Proficiency in administering medications and treatments.
* Strong communication and interpersonal skills.
* Ability to work effectively in a team-oriented environment.
* Willingness to undergo training on restricted health conditions and procedures specific to the program.

Additional Information:

* This is a temporary position covering from May 20th through May 31st (Monday to Friday excluding Memorial Day).
* Prior to the start date, the selected candidate will undergo training on restricted health conditions and procedures for the individuals in the program.
* The specific shift timing is tentatively scheduled from 7:30 AM to 3:30 PM, subject to confirmation.
