The Hospice RN Case Manager is responsible for case management of a group of hospice care clients, providing direct nursing care, and coordinating and supervising other disciplines involved to promote quality services to hospice patients. The Hospice RN is responsible for providing skilled care to their assigned patient according to the plan of care established by the patient's physician. The Hospice RN practices within the scope of nursing as defined by the Nurse Practice Act.

The RN Case Manager works on a team of RNs making PRN/emergent visits; completing admissions; receiving calls from the triage line (Sundays only); has support of both provider and administrator while on call. This is a day shift (8:00am-8:00pm Saturday/Sunday) weekend option job which requires you to meet the needs of hospice patients and their families in their home and/or long-term care facilities. This position requires driving your own vehicle, while receiving mileage reimbursement, to different patient homes and/or facilities to care for them in the Indianapolis area.

Exceptional Skills and Qualifications
  • Compassionate as a professional caregiver.

  • Strong critical thinking and communication skills.
  • Demonstrates ability to respectfully address interpersonal conflicts.
  • Demonstrates customer service skills to provide exceptional patient, and family experience.
  • Graduate of a recognized accredited school of practical nursing, or Graduate from a non-accredited school of practical nursing with three years related professional nursing experience.
  • Licensed as a Registered Nurse (RN) by the Indiana Professional Licensing Agency required.
  • Current Indiana Driver's License required.
  • One year of Hospice or recent inpatient nursing experience required.
