Responsibilities
* This position is the primary liaison for the continuity of care and social support systems for patients and their families throughout their health care experience.
* Develops education and training programs for patients, physicians and care staff on the appropriate clinical, community requirements and programs.
* Guides cancer patients through their course of clinical care working with a variety of medical practitioners, facilities, and community resources.
* Works with community advocates for the patient.
* Tracks and trends data for the program reporting requirements.
* Adheres to policies, procedures and regulations to ensure patient safety and compliance
Qualifications
Job Requirements:
* Bachelor in Nursing or equivalent required.
Preferred Job Requirements:
* Master's in health related field preferred
* 1-2 years Navigator experience preferred, excellent communication skills
