**Responsibilities**

The Pharmacist, Clinical Specialty is responsible for providing specialty pharmacy care services in a remote setting. Assure the appropriate selection, dosing, administering, and dispensing of medication orders in specialized areas, such as nuclear medicine, infectious disease, rheumatology, oncology, cardiology. Serve as a subject matter expert in pharmacy discipline and provide guidance across pharmacies and to clinical leadership.

+ Collaborate with physicians, nurses, pharmacists, and other health care providers as a member of the multidisciplinary team.

+ Communicate with physicians and other clinicians as necessary to facilitate exemplary pharmacy care services to patients.

+ Demonstrate the value of our pharmacy partnership to clinic sites/practices.

+ Complete patient education and comprehensive patient assessments, including medication history, laboratory values, past medical history, pertinent physical assessment, and patient interview.

+ Performs medication therapy and disease state management, which may include critically evaluating disease states and adjusting medication therapy to optimize patient medication regimens and improve clinical outcomes.

+ Collaboratively address financial issues to ensure patient receipt of needed medications

+ Effectively navigate patient prescriptions, including those that are either limited distribution drugs or out-of-network.

+ Collect and maintain accurate data for analysis and financial reporting.

+ Applies the principles of continuous quality improvement consistent with job expectations.

+ Review and assess medication orders to identify, prevent and resolve potential and actual drug related problems.

+ Recommend formulary alternatives when appropriate.

+ Compound and dispense medications for accurate delivery to patients.

+ Provide drug information and education to medical staff and patients.

+ Communicate with physicians and other clinicians as necessary to facilitate care.

+ Oversee technical personnel who assist in the preparation, maintenance, and dispensing of medications and supplies.

+ Assist with academic functions such as serving as a preceptor for pharmacy students.

+ May float to provide coverage of pharmacy needs.

+ Provide patient counseling when required.

**Requirements**

Licensure / Certification / Registration:

+ Pharmacist obtained prior to hire date or job transfer date required. Must be willing to initiate the process to obtain one or more Pharmacist licenses within 90 days of hire/transfer for the following states: AL, AR, IA, ID, KS, KY, LA, MD, MI, MS, NE, OK, OR, TN, TX, VA & WV.

Education:

+ Bachelor's or advanced degree in Pharmacy required.

**Additional Preferences**

+ 3+ years experience in an outpatient, ambulatory clinic setting

+ Specialty medication experience

+ Virtual patient care experience

+ Infectious Diseases Pharmacy Specialty Certification (BCIDP) or Ambulatory Care Pharmacy Specialty Certification (BCACP)
