Overview:

We are seeking a compassionate and dedicated Certified Nursing Assistant (CNA) to join our hospice care team. The ideal candidate will have a genuine passion for providing comfort and support to patients facing end-of-life challenges. As a hospice CNA, you will work collaboratively with our interdisciplinary team to deliver personalized care that meets the physical, emotional, and spiritual needs of our patients and their families.


Responsibilities:


Provide direct patient care under the supervision of a registered nurse (RN) in accordance with the patient's care plan.

Assist patients with activities of daily living (ADLs), including bathing, grooming, dressing, and toileting.

Provide emotional support and companionship to patients and their families, fostering a comforting and supportive environment.

Maintain accurate and timely documentation of patient care activities and observations.

Collaborate with the interdisciplinary team to develop and implement individualized care plans that address the holistic needs of patients and families.

Participate in team meetings, case conferences, and in-service training sessions to enhance your knowledge and skills.

Qualifications:


Valid certification as a Certified Nursing Assistant (CNA) in the state of PA.

Previous experience in hospice, long-term care, or home health care preferred.

Compassionate and empathetic approach to patient care, with a strong commitment to upholding dignity and respect.

Excellent communication and interpersonal skills, with the ability to effectively interact with patients, families, and interdisciplinary team members.

Ability to work collaboratively as part of a multidisciplinary team in a fast-paced and dynamic environment.

Reliable transportation and willingness to travel to patients' homes and nursing facilities.

Job Type: Full-time


Pay: $17.00 - $20.00 per hour


Expected hours: 40 per week
