Registered Nurse - Hospice Acute Care Coordinator - Part Time - Days
GENERAL SUMMARY:
Functions as a care coordinator for Hospice within the hospice setting. Assesses and identifies patient needs in accordance with the hospice concept of care. Coordinates with interdisciplinary team to provide complete patient/family care and referral. Provides pain management and symptom control. Maintains the continuity of care by ensuring patient needs are met.

EDUCATION/EXPERIENCE REQUIRED:
* Requires an Associate's Degree or Diploma in Nursing.
* Requires understanding and acceptance of the Hospice concept of care, and a working knowledge of current pain and symptom management.
* Requires interpersonal skills to present a positive image of hospice to those whom are referred to the program.
* Possess the ability to be flexible in adapting policies and procedures to meet the needs presented by hospice patients and families.
* Previous hospice experience preferred.
CERTIFICATIONS/LICENSURES REQUIRED:
* Requires current licensure in the State of Michigan as a Registered Nurse.
* Hospice certification preferred.
