Registered Nurse Case Manager (Remote) - $45/hour

We are seeking a skilled Registered Nurse Case Manager to join our dedicated healthcare team in a remote, contract-based position at $45 per hour, with high possibilities for extension. This role is ideal for a self-directed professional responsible for facilitating comprehensive care for members with rising health risks or complex healthcare needs, aiming to promote optimal health outcomes. The successful candidate will work within a multidisciplinary team including dieticians, pharmacists, clinicians, and medical directors to assess needs, manage care, and overcome barriers to health.

Responsibilities:
  • Engage members in personalized care plans, coordinating care and services to promote compliance and optimal outcomes
  • Develop and document comprehensive care management plans based on case management criteria and regulatory standards
  • Educate members on medication management and counsel on treatment side effects
  • Maintain ongoing member engagement to support seamless care transitions
  • Collaborate with healthcare providers to gather clinical information and support care plans

Qualifications:
  • 3-5 years of experience in various clinical settings (inpatient, outpatient, or different care levels)
  • Preferred experience in Utilization Management
  • Active RN license in Massachusetts; additional state licensures beneficial
  • Proficiency in multiple IT systems and effective work with diverse populations
