GENERAL SUMMARY

An exempt position responsible for providing care to patients enrolled within TGH Hospital at Home, which is a unique and innovative department at TGH that provides inpatient hospital level care at the patient's home with management of acute illnesses that may or may not be further complicated and/or exacerbated by complex comorbidities. Patient care will be performed in collaboration with an attending physician Hospital at Home Medical Director and multidisciplinary team and include - telehealth, traveling to patient homes, conducting history and physical examinations, managing patients medically with possible minor bedside procedure support and facilitating interventions for patients. APPs exercise autonomous and delegated medical decision making and provide a broad range of diagnostic and therapeutic services including responsibilities for clinical coordination, education, research, and administrative services. In addition to their patient management responsibilities, APP service should include engagement in governance activities, quality / process improvement, ongoing evaluation, and program development for improved clinical outcomes. The acute care Hospital at Home APP will work closely with their Lead APP, Primary Supervising Physician, Service Physicians, Service Medical Director, The Director of Advanced Practice, Advanced Practice Leadership, Service Line Leadership and the operational team. Direct reporting is to the Lead APP and the Manager of Advance Practice Medical, Surgical and Neuroscience Services

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

1. Conducts history/physical examinations, admission workups and medicine reconciliation including assessments and plans in collaboration with supervising physician.

2. Responds to house calls and evaluates patients as needed for pre/post-procedure complications, change in medical condition or other patient/RN concerns.

3. Daily virtual and/or in person rounding on hospital at home patients, reviewing patient data, monitoring therapy and providing continuity of care.

4. Counsels patients/family on condition and pre/post-surgical care.

5. Documents patient information and care in the medical record including H&Ps, progress notes, and discharge summaries.

6. Facilitates discharge planning and patient throughput from admission.

PERFORMING REQUIREMENTS

1. Knowledge of medical model and roles of physicians and APPs. Familiar with anatomy, pharmacology, pathophysiology, clinical medicine, and physical diagnostics.

2. Knowledge of patient assessment techniques including taking histories, performing physicals, evaluating patient condition as pertains to active surgical issues.

3. Knowledge of diagnosing and treating acute / critical care problems and developing care plans.

4. Knowledge of documentation in medical records in a confidential manner for compliance with HIPPA laws and TGH policies.

5. Knowledge of TGH Employment competencies and policies.

6. Knowledge of Medical Staff Bylaws and Rules and Regulations.

WORKING CONDITIONS

Work is primarily performed in patient homes and in the hospital at home Command Center with use of computer technology and remote patient monitoring systems. Frequent travel required between TGH facilities and to/from patient homes will be involved using a TGH provided vehicle. Daily interaction with patients, support team members, physicians, family member, and occasionally external parties (media, public). Lifting, bending, stooping, moderate walking and possible exposure to bodily fluids is involved. On call and weekend scheduling involved.

PA-C or APRN nationally certified as an Acute Care NP or Adult Gerontology Acute Care AP. Hospital medicine experience preferred. Patient care will be performed in collaboration with an attending physician Hospital at Home Medical Director and include - telehealth, traveling to patient homes, conducting physical examinations, managing patients medically with possible minor bedside procedure support and facilitating interventions for patients.

3. Skill in developing/revising patient care plan based on patient status and response to therapies.

5. Skill in interpersonal communication to effectively coordinate, advocate, and deliver patient care.

6. Skill in electronic medical record navigation / documentation.

7. Skill in delivering care via a telehealth platform.

ABILITIES

1. Ability to make responsible decisions within scope of APP practice.

2. Ability to collaborate effectively with physicians and the interprofessional team on complicated cases.

3. Ability to educate patients, families, and staff in a respectful manner.

4. Ability to perform patient assessment techniques to establish an accurate assessment and plan for hospitalized patients at home and at times critically ill patients.

5. Ability to organize, create an efficient schedule allowing for appropriate combinations of APPs for competent coverage.

6. Ability to problem-solve regarding logistical issues that may affect clinical care.

7. Ability to review clinical cases for quality purposes.

QUALIFICATIONS

1. Maintains current education, licensure, registrations, and certifications for specified position held with TGH Advanced Practice that could include either PA-C or AGACNP / ACNP

2. Maintains licensures and registrations in good standing in accordance with State specifications.

3. Continues to develop knowledge and skills with on-going education provided by the TGH People Development Institute.
