Job Summary:

The Minimum Data Set (MDS) Nurse performs under the direct supervision of the Director of Nurses. MDS Nurse attends daily meetings with the interdisciplinary team and weekly Quality Assurance meeting with the medical director and interdisciplinary team. The MDS Nurse shall implement the nursing process in an organized, systematic manner to include resident assessment, planning, intervention, and evaluation as well as documentation in Resident Care Record, completion of Minimum Data Sets, and care plans. MDS Nurse gathers information through resident and staff interview, and the resident's medical record to complete the Resident Assessment Instrument (RAI). The MDS is transmitted on a weekly basis thus allowing timely revenue/reimbursement for resident care. MDS Nurse educates Certified Nursing Assistants on accurate documentation of activities of daily living and develops care plans on admission and updates daily with any changes. MDS Nurse coordinates care plan meetings with families and residents on a regular basis. MDS Nurse is responsible for generating assessment schedules. Provides schedule to all disciplines for completing the Resident Assessment Instrument (RAI) within allowed time limits in accordance with current Federal, State and Local regulations. MDS Nurse oversees the completion of the MDS by all disciplines and transmits completed MDS to CMS within allotted timeframes. MDS Nurse inputs information for the PASSAR (Pre-Admission Screening and Annual Resident Review) to determine if the resident requires a Level 2 and to determine if the resident is appropriate for long term care placement. Generates CASPER reports, case mix rosters, and quality indicator profiles as needed. Submits bed hold records to CMS. Demonstrates the knowledge and skill to assess data reflective of the Resident's status and interprets the appropriate information needed to identify each Resident's requirements relative to needs. Will perform other job duties as assigned.

  • Displays understanding of MDS assessment and care planning process.
  • Proactive, self-motivated approach to researching recent changes to the MDS assessment and documentation process.
  • Organized approach to scheduling MDS assessments and care plan meetings.
  • Displays leadership skills and oversees all charting as it relates to Resident ADL self -Performance.
  • Maintains organized approach to MDS files and record-keeping.

Qualifications:

Education/Skills

Graduate of a recognized non-online LPN program required.

Work Experience:

One year long-term care experience preferred, but not required.

Certification/Licensure-DUE UPON HIRE
