We are seeking a Registered Nurse (RN) Navigator to:

* Bring your experience in pediatrics, pediatric rehabilitation and/or pediatric developmental disabilities.
* Coordinate admissions and assist with coordination of discharges.
* Coordinate family teaching about new rehab conditions, ensure smooth process for family from pre-admission through return to home.
* Collaborate with medical team and/or observe therapy sessions as needed to help determine patient/family education needs.
* Administer and review discharge surveys, lead QI initiatives, and assist with requirements for CARF accreditation.

Education:

Minimum - Bachelor of Science - Nursing degree (BSN).

Preferred - Masters degree in Nursing, Social Work, Public Health, Healthcare Administration, or related field.

Work Experience:

Minimum - Three (3) years of relevant clinical nursing experience.

Preferred - One (1) year of relevant experience in the specific patient population being supported.

Licenses and Certifications:

Minimum - Registered Nurse license in the State of Wisconsin. Basic Life Support/CPR.

Preferred - Applicable Oncology Certification based on patient population.
