If so, Luzerne County Area Agency on Aging has an immediate opening for an Aging Care Manager 2. Our mission is to enhance the quality of life for all mature adults by encouraging independence, promoting healthy lifestyles, and ensuring safety and protection throughout their life journey. DESCRIPTION OF WORK

The employee in this position must have good interviewing and crisis intervention skills as they will conduct field visits in order to determine a level of care decision for the consumer. Field visits include community cases where consumers are living in their homes and facility cases where consumers are living in long-term care settings. They must be able to prioritize their work day in order of importance however, maintain a flexible schedule should they may be called upon to respond to an emergency consumer situation. This employee will need to maintain the knowledge of available community support services for the benefit of older adults and have a working relationship with consumers, outside agencies, the general public, and other staff members. The employee will also need to assist with walk-in consumers for completion of forms, such as PACE, Rebates, Transportation applications, and more. Other duties include but are not limited to completion of required statistical reports, travel vouchers, participation in conference calls, webinars, workshops, and mandatory trainings.

Work Schedule and Additional Information:

+ Full-time employment

+ Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.

+ Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

+ Successful completion of the County Social Casework Intern program;or

+ Six months as an Aging Care Manager 1 or a County Caseworker 1;or

+ Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field;or

+ An equivalent combination of experience and training.
