Job Title: Hospice Registered Nurse (RN) - Per Diem

Job Description:

We are seeking compassionate and experienced Hospice Registered Nurses (RNs) to join our team at UChicago Medicine Ingalls Memorial Hospital on a per diem basis. As a vital member of our hospice care team, you will provide exceptional patient-centered care to individuals nearing the end of life.

Pay Rate: $38/Hr

Shift Details:

• Evening shift: 3:00 pm - 11:30 pm.

Responsibilities:

• Deliver high-quality, holistic care to hospice patients in accordance with established standards and protocols.

• Assess patient needs, develop and implement individualized care plans, and evaluate patient responses to interventions.

• Provide emotional support and guidance to patients and their families during challenging times.

• Collaborate with interdisciplinary team members, including physicians, social workers, and spiritual counselors, to ensure comprehensive care delivery.

• Maintain accurate and timely documentation of patient assessments, interventions, and outcomes in the electronic health record system (EPIC).

Requirements:

• Minimum of 2 years of experience in hospice or a related healthcare setting.

• Current Basic Life Support (BLS) certification.

• Active, unrestricted RN license in the state of Illinois.

• Must provide one supervisor reference within the last 2 years.

• Ability to complete necessary lab work (fit test, physical, MMR, etc.) within 7 days of approval.

Orientation and Training:

• Completion of virtual EPIC training class prior to orientation/shadow shift on the unit.
