This is a per diem/on call position supporting the PACE Irvington site.

The Primary Care Manager RN provides care management, care coordination, skilled care and services for at-risk frail elder patients with complex medical and psychosocial issues. Population-based disease management approaches and application of clinical protocols are integral to the program. This position is a core member of the patient centered PACE interdisciplinary team (IDT).

Required qualifications:

* Upon hire: Oregon Registered Nurse License
* Within 30 days of hire: National Provider BLS - American Heart Association
* 2 years RN experience in PACE, home health, skilled nursing facility, community health, hospice nursing or related experience

Preferred qualifications:

* Bachelor's Degree in Nursing
* Upon hire: Current recognition by a national professional organization in care management, geriatrics or hospice
