Staff Physician, Post-Acute and Long Term Care

Hackensack University Medical Center - Hackensack, New Jersey

Hackensack Meridian Health is seeking a dedicated Post-Acute and Long Term Care Staff Physician to join our team at Hackensack University Medical Center. The successful candidate will be responsible for all professional, administrative and quality improvement activities within the Post-Acute and Long Term Care setting.


RESPONSIBILITIES:

  • Provide direct medical care in post-acute and long-term care facilities.
  • Fulfill administrative responsibilities and oversee quality improvement and regulatory compliance as medical director in the post acute and long term care setting.
  • Develop and implement appropriate advanced disease treatment plans that effectively manage the patient’s needs creating a culture with optimal utilization of resources.
  • Communicate effectively with a multidisciplinary team to ensure seamless patient care and transitions.
  • Actively engage in continuing medical education programs and lead post-acute care and long term care education for trainees, including geriatric fellows, internal and family medicine residents, medical students and other interdisciplinary students.
  • Collaborate with the Section Chief in the design and implementation of teaching and education programs in post acute and long term care staff.
  • Participate in the design and implementation of education programs for medical and other staff, as well as education programs for the community.

REQUIREMENTS:

  • Degree from a recognized medical school
  • Have or have the ability to obtain a New Jersey Medical License
  • Board-certified/Board-eligible in Internal Medicine/Geriatrics.
  • CMD certification preferred
  • Experience in internal medicine/ geriatric medicine and/or an affiliation with an academic institution
  • Ability to relate effectively to all levels of the clinical and administrative structure of the medical center as well as of other institutions affiliated with the Division of Geriatrics.
