PUBLIC HEALTH NURSE

This position is funded by a grant for a duration of time as defined by the grantor.
Perform routine public health nursing duties in the Health Department's Family Planning Program. Provide care coordination that contributes to optimum reproductive health outcomes.
Pay Grade: 52H
This position is eligible for a HIRING BONUS outlined below:
1st payment of $1,500 paid after completion of six (6) months of service,
2nd payment of $1,500 paid after completion of twelve (12) months of service,
3rd payment of $2,000 paid after completion of eighteen (18) months of service.
Minimum Qualifications
100% -
1. Two (2) years of generalized registered nursing experience; and AD/Diploma from an accredited School of Nursing; OR
2. Bachelor's degree from an accredited college or university in nursing.
3. Active, unrestricted license as a Registered Nurse in the State of Tennessee (OR application for license transfer to TN must be in process for consideration).
4. Reproductive health or women's health or outpatient clinic experience preferred.
5. PROOF OF EDUCATION, TRAINING AND/OR EXPERIENCE IS REQUIRED.
Duties and Responsibilities
1. Assesses and monitors for reproductive health concerns using nursing process and program protocol, including assessment of individual risk and presenting health-related conditions following Tennessee Department of Health, Centers for Disease Control (CDC) and Family Planning Program Standards.
2. Assesses using appropriate diagnostic and treatment procedures (under medical direction), and follow up care while maintaining medical safety standards.
3. Implements specified diagnostic and treatment procedures and program strategies under family planning protocol, public health guidelines and practices for patients of reproductive age.
4. Conducts follow-up or case management such as by telephone or letter to assure quality care.
5. Investigates clinical issues/concerns relative to presenting risk or health-related condition.
6. Implements protocol-specific diagnostic and treatment procedures and data analyses and participate in quality improvement activities of program or unit.
7. Participates in development and implementation of program plans for educating and counseling individuals (in varied settings) for prevention of disease and maintenance of reproductive health.
8. Informs through documentation of accurate, appropriate data in medical records systems and report to supervisor regarding relevant data to assure that standards of care are followed in accordance with protocol, CDC, Family Planning Guidelines, legal requirements and program guidelines.
9. Communicates during team meetings and consult with the Family Planning providers, as necessary, and inform supervisor of barriers interfering with health care assessment and provision.
10. Mobilizes linkages between the SCHD and community medical providers specified by protocol to assure reproductive health needs are met.
11. Participates in activities including in-service training programs to assure competence of new and existing employees and students, local lay and official groups and agencies to promote an integrated public health service, to raise understanding of the public health team in provision of services to the community.
12. Provides oversight for small and/or specialty area or staff of lower classification, as assigned such as assisting supervisor with drug and/or medical supply inventory, representing supervisor at meetings and communicating appropriate information to staff.
13. Performs other related duties as required or directed.

10. Skilled in the use of personal computers and related software applications.
11. Ability to effectively manage time, organize, implement, and maintain a tracking system to ensure clients' needs are met.
Special Requirements
1. Must possess a valid Tennessee driver license by date of employment.
2. Must have daily access to an operable automobile and meet county requirements for automobile insurance upon date of employment.
3. Must be flexible and available to work some nights and/or weekends.
4. Licensed or eligible for licensure in the State of Tennessee (Licensure application must have been submitted upon date of employment).
5. Must maintain licenses and certifications throughout duration of employment.
