Job Description:

Essential Duties and Responsibilities:

* Perform comprehensive health assessments on patients of all ages
* Diagnose and treat a wide range of medical conditions, including chronic diseases such as diabetes, hypertension, and heart disease
* Order and interpret diagnostic tests
* Prescribe medications and treatments
* Develop and implement individualized treatment plans
* Educate patients about their health conditions and medications
* Coordinate care with other healthcare providers, such as specialists and social workers
* Participate in patient care quality improvement initiatives
* Maintain accurate and complete patient records, EPIC EMR

Qualifications:

* Master's degree in nursing with a focus on advanced practice nursing (APRN)
* Certified as an APRN by the American Nurses Credentialing Center (ANCC) in internal medicine
* Current license to practice as a registered nurse in the state of NH
* Experience in providing primary care to adults of all ages
* Excellent communication and interpersonal skills
* Ability to work independently and as part of a team
