The Nurse Navigator will work collaboratively with the faculty experts in Hematologic Malignancies. The NN also works with Knight Cancer teams to include providers from hem medical oncology, surgical oncology, surgery, radiation oncology as well as RNC, CC, and TC for the care of hematologic malignancy patient population. The nurse navigator coordinates the care for the patient prior to the patient's first consult visit at OHSU, and in some cases, through the work up and diagnosis phase of care. This position will work with the clinical nurse manager and other ambulatory oncology nurse navigators to develop standard metrics, identify patient barriers, coordinate smooth transitions of care, and to be available for team members in gathering data for new patients that will allow the team to create a plan of care at the patient's first visit and, through the work up phase of their disease. This position is part of the hematology/hem malignancy cluster within the Nurse Navigator team and provides cross coverage as needed for navigating patients for new consults with disease expertise in leukemia, lymphoma, myeloma, transplant, CAR-T therapy.

An Oncology Nurse Navigator (ONN) at OHSU is a professional RN with oncology-specific clinical knowledge who offers individual assistance to patients, families, and caregivers to help overcome healthcare system barriers. Using the nursing process, an ONN provides education and resources to facilitate informed decision-making and timely access to quality health and psychosocial care as they approach their first visit. This individual will serve as the primary point of contact for all navigated patients within the disease specialties they cover and will thoroughly hand off care (verbally and in EMR documentation) to providers and nurses assuming care of the patient after the first consult.

The ONN interfaces with research staff, and works to identify patients who may be suitable candidates for clinical trials.
