Overview:
We are seeking a compassionate and skilled Hospice Nurse to join our team. As a Hospice Nurse, you will provide specialized care and support to patients and their families during end-of-life care. This is a rewarding opportunity to make a difference in the lives of those facing terminal illnesses.

Responsibilities:
- Conduct comprehensive assessments of patients' physical, emotional, and spiritual needs
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Administer medications and treatments as prescribed
- Monitor patients' conditions and provide ongoing support and comfort
- Educate patients and families on disease progression, symptom management, and end-of-life care options
- Coordinate with other healthcare professionals to ensure continuity of care
- Provide emotional support to patients and their families throughout the hospice journey
- Assist with discharge planning and coordination of services post-hospice care

Experience:
To be successful in this role, you should have the following experience and qualifications:
- Minimum of 2 years of nursing experience in a healthcare setting
- Experience in critical care, trauma medicine, or cardiac catheterization is highly desirable
- Knowledge of medical terminology and proficiency in using electronic medical record systems (Epic)
- Familiarity with hospice palliative medicine principles and practices
- Experience in infant care and neonatal ventilator management is a plus

- Strong communication skills to effectively interact with patients, families, and interdisciplinary team members
- Compassion and empathy to provide emotional support during difficult times
- Excellent critical thinking and problem-solving abilities
- Ability to work collaboratively as part of a team while also being self-directed
