About the opening:
- Contract position with potential for extension
- Fully remote role offering $45 per hour
- Integral part of a multidisciplinary team including medical and behavioral health care managers
Responsibilities:
- Engage members in tailored care plans, ensuring communication and education
- Document and manage comprehensive care plans following case management standards
- Educate members on medication and treatment side effects
- Foster ongoing member engagement for seamless care transitions
- Collaborate with healthcare providers to enhance care plan support
- Monitor and escalate clinical quality issues as needed
- Provide guidance on insurance products and compliance with health regulations
Qualifications:
- Active RN licensure in Massachusetts; additional licensure beneficial
- Residency in Massachusetts required
- 3-5 years experience in diverse healthcare settings
- Utilization Management experience preferred
- Proficient in various IT systems
- Effective in multicultural and diverse settings
- Independent, adaptable, team-oriented professional
