JOB SUMMARY

Pharmacotherapy Specialists in Ambulatory Transplant Clinics serves as an integral part of the dynamic team of individuals who support Transplant Services. Pharmacotherapy Specialists are educational resources, participates and leads team development for transplant pharmacy, are involved in formulary management, including design and completion of medication use evaluations, and guideline development. Flexibility and competence to cross cover in other areas of solid organ transplantation specialty is necessary.

Under general supervision, and according to established polices and procedures, responsible for the provision of specialized clinical pharmacy services to patients, nurses and medical staff. Collaborates with other health care professionals in the research, development and implementation of critical pathways and policies that have a medication use component. Practice involves the components of expert practitioner, educator, consultant, researcher, and clinical leader. Responsible for performing job duties in accordance with the mission, vision and values of Tampa General Hospital.

* Pharmacy degree (Pharm. D or Ph.D.) from an accredited college of pharmacy
* Completion of PGY2 Residency or fellowship in Solid Organ Transplant or at least 3 years of clinical pharmacy experience in transplant specialty areas
* Current license as a Registered Pharmacist in the state of Florida.
