Efforts historically have targeted narrow programs and patient populations, and while many have succeeded, healthcare costs continue to rise. It’s time to take a radically different approach. A new approach involves putting the patient at the center with the goals of improving value and providing better outcomes at lower cost for patients. The Population Health Management (PHM) department at Mass General Brigham is charged with the challenge to drive better value for patients across Mass General Brigham. To achieve its goals, PHM leverages financial and clinical data to examine opportunities, designs and delivers innovative care models using product development and design thinking disciplines, and leverages problem solving, teamwork, and leadership skills to drive ongoing improvement.Reporting to a Program Manager for Care Management, the Project Specialist for Care Management Analytics provides analysis support and contributes to the development of recommendations for new interventions, identification of key program drivers, supports Mass Health contract data requirements, and provides data to support program improvement and other projects as needed.The Specialist is involved in multiple projects that answer broad questions about care management program performance and explore new program opportunities through analysis As part of a highly collaborative team, the Analytic Specialist works closely with the Program Director and Program Managers.Produce routine standard reports and contract deliverables, review to ensure data accuracy and troubleshoot, as appropriateComplete quantitative and qualitative analyses to support the objectives of assigned projects. This includes creating charts, graphs, and transforming information gathered into a format that is clear, accurate, and useful for facilitating decision-making.Support the development of key program questions to frame analytics; ensure data requested and analyses conducted answer the critical questions such as key program performance drivers and program expansion opportunities.Assist with ongoing efforts to track, report, and evaluate high risk member management efforts using multiple data sources including claims and non- claims-based data.Act as subject matter expert in using claims and EPIC data from the Enterprise Data warehouse (EDW) to provide both business knowledge and analytical support to the Care Management teams. Work to improve visualization of current reports using existing tools and new tools as they become available in EDW environment.Assist with presentations and updates of findings, and make recommendations to internal and external stakeholders/leaders as needed for assigned projectsWork collaboratively with Program Director, Managers, and Project Managers in designated content areaSpecial projects as assignedUse/s the Mass General Brigham values to govern decisions, actions, and behaviors.
