Responsibilities:

* Deliver exceptional nursing care to patients in the comfort of their homes, ensuring their safety, comfort, and overall well-being.
* Administer medications and treatments as prescribed by healthcare professionals, adhering to the highest standards of accuracy and patient safety.
* Monitor patients' vital signs, observe any changes or concerns, and communicate effectively with the healthcare team.
* Collaborate with physicians, therapists, and other healthcare providers to develop individualized care plans that meet the unique needs of each patient.
* Educate patients and their families on healthcare procedures, medication management, and strategies for disease prevention and self-care.
* Document patient progress, observations, and interventions promptly and accurately, maintaining meticulous records.
* Uphold infection control protocols and maintain a clean, organized, and welcoming environment.
* Provide emotional support, companionship, and encouragement to patients and their families during their healthcare journey.

Requirements:

* Current and valid LPN license in the state of Florida.
* Minimum of 1 year of clinical experience as an LPN, preferably in a home health setting.
* Excellent communication, interpersonal, and critical thinking skills.
* Ability to work independently and collaboratively within a multidisciplinary team.
* Possession of a valid driver's license and reliable transportation.
