PURPOSE:

The Hospice Registered Nurse (RN), Per Diem is responsible as a member of the interdisciplinary team under the supervision of the Medical Director and the patient's attending physician. The Hospice RN will provide skilled nursing assessment, planning, and care to maximize the comfort and health needs identified by the patient/family unit. Additionally, they will coordinate services to provide preventive and supportive care to the terminally ill and their families in accordance with approved care plans and Hospice policies and procedures. This position will also demonstrate commitment to the philosophy, goals, and objectives of the hospice concept by utilizing teaching, assessment, and intervention skills to maximize the quality of life of patients and families in the patient's home setting. As needed, the Hospice RN will direct the work of the LPN and. CNA.

At Tri-Cities Chaplaincy, we carefully establish salary ranges for our positions by taking various factors into account, including education, certifications, knowledge, skills, experience, and market conditions. These ranges are tailored to match the diverse programs and services we offer. Our goal is to ensure fair compensation for candidates based on these factors while maintaining internal peer equity.

Pay Range: $35.50/hr - $65.79/hr

Mileage Reimbursement: $0.67 per mile, for eligible positions.

Sick Time: Accrues in accordance with WA state law

Per Diem Pay: Per diem pay of an additional $2.00/hour

Requirements

MINIMUM POSITION QUALIFICATIONS
  1. Current and unrestricted Washington state registered nurse (RN) license.
  2. At least two years of nursing experience, preferably with one year of home health or hospice nursing.
  3. Available to work two shifts per month, one must be a night or weekend shift, and a minimum of two annual holidays per year to maintain per diem status.
  4. Must be willing to train and float to in-home, hospice house, admissions, and other clinical areas, as needed.
  5. Intermediate computer skills.
  6. This position is designated as safety-sensitive and will require pre-employment drug testing, which includes cannabis testing in WA State.

CLINICAL STAFF POSITION QUALIFICATIONS
  1. As required per position/credentialing; successful completion of Blood Born Pathogens Training
  2. Certification in Cardio-Pulmonary Resuscitation (CPR)/Basic Life Safety (BLS)
  3. (Hospice House) Washington State Food Worker Card issued by the Department of Health

RESPONSIBILITIES AND ESSENTIAL FUNCTIONS
  1. Initiate communication with attending physician(s), other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/family.
  2. Maintain regular communication with the Clinical Nursing Supervisor to review patient needs.
  3. Maintain regular communication with the Medical Director and attending physician concerning patient/family care needs and response to interventions.
  4. Maintain up-to-date patient records so that problems, plans, actions, and goals are accurately and clearly stated, and changes are reflected as they occur.
  5. Instruct primary caregivers, volunteers, and employed caregivers to provide care as indicated.
  6. Inform the Clinical Nursing Supervisor of unusual, or potentially problematic, patient/family issues.
  7. Provide appropriate support at the time of death
  8. Participate in the Tri-Cities Chaplaincy's orientation and in-service training programs for professional staff.
  9. Must be able to perform admissions for both In-Home and Hospice House, informationals, and nursing visits.
  10. Participate in agency and community programs as requested to promote professional growth and understanding of hospice care.
  11. Demonstrate familiarity with policies of the Tri-Cities Chaplaincy, rules and regulations of Washington State, and Federal regulations that aid in determining policies.
  12. Accept other assignments as appropriate.
  13. Participate in new staff orientation as requested by Clinical Nursing Supervisor.
  14. All Tri-Cities Chaplaincy employees will follow the minimum standard regarding PHI when accessing patient information on a need-to-know basis only. All employees will practice appropriate safeguards when accessing this information.
  15. Able to cover on-call after-hours, weekend, and holiday shifts.
  16. Ability to accept a wide variety of assignments and adjust to changing situations.
  17. Ability to read and write clearly, concisely, and legibly.
  18. Ability to work cooperatively with others and function as a member of a team.
  19. Ability to follow written and oral instructions.
  20. Ability to communicate effectively with and respond appropriately to patients and families in both routine and crisis situations.
  21. Ability to maintain a neat and clean appearance in accordance with agency dress policy.
  22. Ability to perform personal patient care and maintain a mature, sensitive, and caring attitude toward patients and their families.
  23. Excellent triage skills
  24. Ability to work at a designated worksite location.
  25. Perform other duties as assigned.

AGENCY REQUIREMENTS
  1. Demonstrate support and adherence to the agency's Mission, Vision, and Values.
  2. Participate in community Education and Outreach to raise awareness on agency provided programs including but not limited to hospice care, advanced care planning and bereavement.
  3. Responsible for knowledge and compliance with agency policies, procedures, and practices.
  4. Responsible for maintaining and protecting Patient and agency information in a confidential manner. Practice appropriate HIPAA confidentiality by accessing patient-related information on a need to know basis only.
  5. Reliable personal transportation, and current and valid driver's license and vehicle insurance.
  6. Satisfaction of state and agency requirements for Tuberculin screening.
  7. Hepatitis B immunization series, proof of series, or signed declination.
  8. Proof of applicable Vaccination(s) on file or approved accommodation/declination as appropriate on file.
  9. Must pass criminal background screening and pre-employment drug screening.
  10. Ability to meet the physical demands of the position (see attached physical requirements and environmental conditions).
  11. Complete agency and departmental orientation and training program(s).
  12. Complete all continued learning, education, policy reviews, and required training by their respective deadlines.
  13. Maintain licensure(s) and certification(s) required for the position.
  14. Available to work as assigned/scheduled.
  15. Perform other duties as assigned.
