The position is fully remote, but candidates must live in the Nashville and/or surrounding areas. We have quarterly in person meetings. Previous experience with the Medicare population is preferred. Care/Case Management experience preferred. Experience in patient education and disease management is preferred. Technologically agile, multi-tasking in multiple online platforms and portals

Responsibilities

Serve as a patient navigator throughout the continuum of care.

* Function as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers.
* Coordinate efforts for treatment and promote quality improvement and educational efforts.
* Compile data, track outcomes, and make recommendations for process improvement.
* Provide referrals/support for follow-up care for patient who are uninsured or underinsured.

* Connect patients and families to community and social support services. Assist patients in finding ways to pay for their healthcare as needed. Assist in helping plan and implement oncology outreach screening program and classes.
* Facilitate interaction and communication with healthcare staff and providers. Identify personnel in departments involved in the care of oncology patients. Provide support through empathetic listening. All other duties as assigned
* Use interventions and strategies that are appropriate to the individual and the population i.e. taking into account culture language age and gender.

Requirements

Licensure / Certification / Registration:

* Licensed Registered Nurse credentialed from the Tennessee Board of Nursing OR current home state license for multi-state license recognition Compact State obtained prior to hire date or job transfer date required.

Education:

* Diploma from an accredited school/college of nursing OR Required professional licensure at time of

hire.
